1. Photo of the registered optometry practitioner
2. Name and the Identity Card number of the registered optometry
practitioner
3. Type of registered optometry practitioner (Optometrist
or Optician)
4. Registered optometry practitioner registration number
5. Current annual practicing certificate number
6. Contact lens permit number (Opticians only)
7. Scope of practice of the registered optometry practitioner
8. Name and address of the practices (limit to 5 addresses)
9. Date of Issuance, Signature of the Registrar and endorsement
